What to know BEFORE you book.

I take patients by appointment only. my practice is in the front of my home. absolutely no walk-ins!

your booking time is for your entire appointment, including intake, treatment, and home care. please complete all forms in advance and arrive on time to ensure maximum treatment time. my general recommendation is 45 min – 1-2 body areas, 60 min – 2-3 body areas, 90min – 4+ body areas. 30 min – acute injury (e.g. slept funny and lost range of motion in your neck). I do not do spa-style full-body Swedish massage.

I use essential oils in my practice, so while my space is not scent-free please do not wear strong perfumes, body sprays, or other products.

*CURRENTLY NOT ACCEPTING NEW PATIENTS EXCEPT BY DIRECT REFERRAL* if a friend has referred you please let me know!

please email me at claire@mindbodyself.ca with your referral details BEFORE booking an appointment

insurance

I cannot direct-bill to any insurance companies. Registered Massage Therapy is covered by most insurance plans; however, it is your responsibility to review and confirm your coverage. I perform acupuncture under my RMT licence.

cancellation/no show

24-hour notice is required to reschedule or cancel an appointment. it is your responsibility to turn on notifications and reminders when creating an account, and you can cancel prior to the 24 hour period from the booking site.

if you cancel, reschedule, or no-show for your appointment for any reason with less than 24 hours notice, you will be charged 100% of the treatment cost which must be paid prior to booking future appointments.

you can avoid the fee by swapping with a friend/family member who is an existing patient of mine but it is your responsibility to find someone. a minimum of 6 hours notice is required for any patient swaps.

repeated, regular cancellations and patient swapping may result in the termination of future services.

illness

while the world has moved away from COVID precautions, I still cannot treat you if you are sick with anything. it’s not safe for me nor is it beneficial to you.

you will receive a MANDATORY illness screening 24 hrs in advance of your appointment. this gives you a chance to assess your health and make sure you are well enough for your appointment in time to avoid cancellation charges.

if you late cancel due to illness (minimum 6 hours notice) you will be charged 50% of the treatment cost which must be paid prior to booking future appointments. see cancellation policy for other details.

health & safety

I wear n95 masks while treating and will continue to do so for the foreseeable future. I will not require patients to wear masks, however, I ask that you wear a mask if you have recently been sick or have recently been around someone who is sick.

this policy may change to required masking during the height of cold & flu season, please check in about masking if you have any concerns.

late arrival

your booking time is for your entire appointment, including intake, treatment, and home care. please complete all forms in advance and arrive on time to ensure maximum treatment time. I cannot accommodate additional time to make up for late arrival and cannot adjust your appointment to a shorter time because of late arrival.

refusing service

I center the safety and comfort of my high-risk patients and those with historically marginalized identities:

hate speech or aggressive behaviour of any kind will not be tolerated

racism, transphobia, homophobia, sexism etc. will not be tolerated

intoxication of any kind will not be tolerated

inappropriate or sexual behaviour, including speech, will not be tolerated

refusal to comply with policies and protocols, including health & safety measures, will not be tolerated

privacy

appointments MUST be booked by the person receiving the treatment and in that person’s name. I cannot discuss the details of any patient with anyone else, including family members, without their written permission.

I am a registered healthcare practitioner and am legally required to obtain a thorough health history form prior to treatment (to be renewed annually). all information is kept private and confidential. your written permission is required to release such information and will only be released if required by law. my governing body is the College of Massage Therapists of Ontario and massage therapists are regulated under the Massage Therapy Act and the Health Care Act.

to provide safe and effective treatment I may discuss the general aspects of patient treatment plans without any identifying information (e.g., to discuss new treatment approaches for hip impingement with a colleague). case study discussion is limited to other health care professionals and contains no personal information.

if you wish to create a circle of care relationship between health care providers (i.e., communication with your doctor, psychiatrist or other) your written request is required to all professionals.
